Medical Statement

Your child's immunization records is required by New York State. This form must be completed and submitted at the time of registration.

Policy

You can review the Smiley Faces Policy Form and let us know what days and hours of care you would like us to provide for your child. This form must be completed and submitted at the time of registration.

Medication Consent Form

Must be completed by parent and physician. For prescribed medication and for over-the-counter medication

Non-Medication Consent Form

Must be completed by parent. For over the counter products.

Individual Allergy and Anaphylaxis Emergency Plan Form

Must be completed by parent and physician.
