Contract

The Smiley Faces Enrollment Contract / Parent Handbook which details important information must be completed and submitted at the time of registration.
       

Registration


The Smiley Faces Registration Form will provide us important information about your child's emergency contact information and other details to help us provide the best possible care. This form must be completed and submitted at the time of registration.
 

Medical Statement

The Smiley Faces Medical Statement Form gives us details about your child's immunization records and is required by New York State. This form must be completed and submitted at the time of registration.
 

Policy

You can review the Smiley Faces Policy Form and let us know what days and hours of care you would like us to provide for your child. This form must be completed and submitted at the time of registration.
